Implica la coordinaci\u00f3 entre Educaci\u00f3, Salut i Drets Socials per garantir una resposta integral.<\/p>\n\n\n\n<p class=\"has-text-color has-medium-font-size\" style=\"color:#000000\">\ud83d\udcdd <strong>Activaci\u00f3 de la tercera fase<\/strong><br>Requereix una sol\u00b7licitud formal al Servei Territorial, amb una descripci\u00f3 de la situaci\u00f3 i
