{"id":9944,"date":"2020-02-17T13:16:16","date_gmt":"2020-02-17T12:16:16","guid":{"rendered":"http:\/\/serveiseducatius.xtec.cat\/altemporda\/?page_id=9944"},"modified":"2025-10-14T12:24:50","modified_gmt":"2025-10-14T10:24:50","slug":"eap","status":"publish","type":"page","link":"https:\/\/serveiseducatius.xtec.cat\/altemporda\/el-servei-educatiu\/equip\/eap\/","title":{"rendered":"EAP"},"content":{"rendered":"<p>Els <strong>EAP<\/strong> s\u00f3n <strong>equips d&#8217;assessorament i orientaci\u00f3 psicopedag\u00f2gica<\/strong> que donen suport al professorat i als centres educatius per a l&#8217;atenci\u00f3 educativa de l&#8217;alumnat, principalment el que presenta necessitats espec\u00edfiques de suport educatiu, i a les seves fam\u00edlies.<\/p>\n<h5>Els professionals de l&#8217;EAP s\u00f3n:<\/h5>\n<p>\u00b7 Docents d&#8217;ensenyament secundari amb l&#8217;especialitat de psicologia i pedagogia<\/p>\n<p>\u00b7 Treballadores socials<\/p>\n<p>\u00b7 Fisioterapeutes<\/p>\n<h4><\/h4>\n<h4>FUNCIONS<\/h4>\n<p><strong>Psicopedagogues:<\/strong><\/p>\n<ul>\n<li>Avaluaci\u00f3 psicopedag\u00f2gica, motriu i social en el context escolar a proposta de la CAEI per a:\n<ul>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Orientar la resposta educativa<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Tenir un informe de reconeixement NESE derivats de NEE, trastorn que condiciona l\u2019aprenentatge, altes capacitats, retards greus d\u2019aprenentatge i situacions socioculturals i socioecon\u00f2miques desafavorides.<\/span><\/li>\n<\/ul>\n<ul>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Derivar a algun servei espec\u00edfic del Departament d&#8217;Educaci\u00f3: CREDAG, CREDV, CRETDIC,&#8230;<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Requerir algun suport complementari (vetlladors) i\/o equipament espec\u00edfic.<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Plantejar un suport intensiu (CEE, SIEI, SIAL,CEEPSIR, UEC&#8230;).<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Ser derivats a altres serveis externs: pediatria, CDIAP, Serveis Socials, serveis psicopedag\u00f2gics privats&#8230; <\/span><\/li>\n<\/ul>\n<\/li>\n<li>Seguiment de l&#8217;alumnat amb necessitats espec\u00edfiques de suport educatiu un cop detectades les dificultats, en coordinaci\u00f3 amb els serveis externs implicats i a proposta de la comissi\u00f3 d&#8217;atenci\u00f3 educativa inclusiva (CAEI) i\/o de la comissi\u00f3 social del centre.<\/li>\n<li>Elaboraci\u00f3 de l\u2018informe de l&#8217;EAP de reconeixement de necessitats espec\u00edfiques de suport educatiu per a la previsi\u00f3 de suports i proposta d&#8217;escolaritzaci\u00f3 per a l&#8217;alumnat amb necessitats educatives especials.<\/li>\n<li>Assessorament en l&#8217;elaboraci\u00f3 del pla de suport individualitzat (PI) de l&#8217;alumnat amb informe de reconeixement de necessitats espec\u00edfiques de suport educatiu.<\/li>\n<li>Col\u00b7laboraci\u00f3 i acompanyament al llarg de tota l&#8217;escolaritat, en el pas de curs, en la transici\u00f3 entre etapes i centres educatius de l&#8217;alumnat amb necessitats espec\u00edfiques de suport educatiu, i en l&#8217;orientaci\u00f3 per a la continu\u00eftat formativa i\/o l&#8217;acc\u00e9s al m\u00f3n laboral.<\/li>\n<li>Assessorament en la concreci\u00f3 de mesures de detecci\u00f3 i d&#8217;atenci\u00f3 educativa a l&#8217;alumnat, i en l&#8217;avaluaci\u00f3 de la idone\u00eftat de les mesures i suports per a l&#8217;organitzaci\u00f3 del centre, en col.laboraci\u00f3 amb el professorat especialista i els serveis educatius espec\u00edfics (CREDAG, CRETDIC, CREDV&#8230;).<\/li>\n<\/ul>\n<p>&nbsp;<\/p>\n<p><strong>Treballadores socials:<\/strong><\/p>\n<ul>\n<li>Identificaci\u00f3, avaluaci\u00f3 i seguiment de les necessitats educatives de l\u2019alumnat vinculades a l&#8217;entorn social i familiar<\/li>\n<li>Assessorament i informaci\u00f3 als centres educatius sobre la identificaci\u00f3 d\u2019alumnat amb necessitats espec\u00edfiques de suport educatiu\u00a0 derivades de risc social i col\u00b7laboraci\u00f3 en la planificaci\u00f3 d&#8217;actuacions que afavoreixin el seu desenvolupament i aprenentatge en els contextos escolar i social<\/li>\n<li>Informaci\u00f3 i assessorament a les fam\u00edlies en relaci\u00f3 a l\u2019atenci\u00f3 dels seus fills\/es i sobre serveis i recursos existents en els diferents moments de l&#8217;escolaritzaci\u00f3<\/li>\n<li>Coordinaci\u00f3 i col\u00b7laboraci\u00f3 amb els serveis i institucions de la zona per donar resposta coherent a les necessitats educatives de l&#8217;alumnat des d\u2019un punt de vista integral<\/li>\n<\/ul>\n<p>&nbsp;<\/p>\n<p><strong>Fisioterapeutes:<\/strong><\/p>\n<ul>\n<li>Valoraci\u00f3 motriu de l&#8217;alumnat, tenint en compte el diagn\u00f2stic m\u00e8dic si n&#8217;hi ha, per descriure el perfil de funcionament individual, posant \u00e8mfasi en les fortaleses i analitzant els factors contextuals que suposen un obstacle en la participaci\u00f3 i el funcionament de l\u2019infant o jove<\/li>\n<li>Assessorament al professorat en el pla individualitzat de l&#8217;alumnat sobre mesures i suports per afavorir l\u2019acc\u00e9s als aprenentatges en condicions d\u2019equitat i poder actuar de forma directa sobre els diferents obstacles (barreres d\u2019acc\u00e9s a la comunicaci\u00f3, a materials, organitzatives de centre, personals, socials, arquitect\u00f2niques, etc.).<\/li>\n<li>Informaci\u00f3 i assessorament a les fam\u00edlies dels alumnes sobre les necessitats i evoluci\u00f3 dels seus fills\/es<\/li>\n<li>Coordinaci\u00f3 amb altres professionals, especialistes i\/o altres serveis.<\/li>\n<\/ul>\n<h4><\/h4>\n<p>&nbsp;<\/p>\n<h4>HORARI DEL SERVEI:<\/h4>\n<p>Atenci\u00f3 directa als centres educatius: de 9 h a 14 h de dilluns a divendres<\/p>\n<p>Atenci\u00f3 al p\u00fablic al Servei Educatiu: dijous 15 a 18h<\/p>\n<p>Tel\u00e8fon: 972511814\u00a0 &#8211;
